Blair County Community Health Needs Assessment

To build a healthier community in Blair County, we are conducting a Household Survey in collaboration with community partners to learn more about strengths and issues in neighborhoods and households. Everyone’s feedback will help improve all aspects of a healthy Blair County (e.g. social, economic, physical, emotional, etc.). Your responses will be combined with others providing us with information on what is most important so our work groups can develop programs and activities that are most beneficial.

We would like an adult (18 years of age or older) in your household to complete this survey as soon as possible, but no later than August 1, 2024. We are not tracking who completed the survey and there is no way to identify you or your household.

When you are completing this survey:
Community refers to your municipality, township, borough, or city.
Household includes you and members of your family and others living in your house.

We have completed our 2021 community health needs assessment. This report includes the results of this assessment, a summary of trends from past assessments, accomplishments, and future programs/activities as part of the Implementation Plan.

The Healthy Blair County Coalition (HBCC) has completed the 2018 community health needs assessment. The information gathered from the needs assessment will be used by the HBCC Steering Committee and many organizations to develop or enhance services and programs throughout Blair County.

Ever since the needs assessments reports were published, our community has been taking action to address many of the issues discovered in the research. Please review the following report which provides an overview of our goals, action steps, and accomplishments.

Beginning in 2012, all hospitals were required by the Patient Protection and Affordable Care Act to conduct a community health needs assessment every three years and to develop an intervention plan to meet community health needs.

In 2007, the United Way of Blair County and the Blair County Human Services Office invited organizations to collaborate on a community-wide needs assessment.
